About the role
The Care Coordinator is the consistent voice families hear most often at Monarch. You'll keep the moving parts of each household's plan in sync: bookings, family check ins, specialist follow ups, caregiver scheduling, and the steady documentation that makes the rest of the clinical team's work possible.
Responsibilities
- Maintain each household's master schedule and confirm visits in advance.
- Be the named contact for families: first call, last call, and most of the calls in between.
- Book and follow up on specialist referrals; chase results and ensure they land in the plan.
- Coordinate caregiver staffing across Home Care households, including overnight rotations.
- Keep the household record current: plans, contacts, shift notes, communications.
- Flag clinical changes to the nurse and physician so issues are reviewed early, not late.
Requirements
- Three or more years coordinating in healthcare, home care, social services, or a related people heavy operations role.
- Excellent written and verbal communication, as you'll be the family voice of the practice.
- Comfort with scheduling tools and structured documentation.
- Eligibility to work in Canada.
Nice to have
- Direct experience with senior care or family caregiving.
- Comfort handling sensitive conversations with families navigating decline.
- Familiarity with PIPA / PIPEDA privacy obligations.
