We're hiring a Registered Nurse to lead in home clinical work for our Ageing in Place and Home Care programs. You'll work alongside a Monarch physician and a small care coordination team, conducting comprehensive assessments, building plans with families, and providing the steady follow up that keeps proactive care actually proactive.

What you would do

  • Conduct comprehensive in home clinical and functional assessments.
  • Lead medication reviews and reconciliations with the client's pharmacy.
  • Document and update written care plans in the household record.
  • Coordinate with the family physician, specialists, and the client's family, with consent.
  • Provide ongoing nursing follow up at the cadence each plan calls for (typically monthly or quarterly).
  • Mentor caregivers staffed on the household and review shift documentation.

What we are looking for

  • Active registration in good standing with the BC College of Nurses and Midwives (BCCNM).
  • Current professional liability coverage.
  • Three or more years of relevant nursing experience (community, home care, geriatrics, or primary care).
  • Valid BC driver's licence and reliable vehicle (visits across Greater Victoria).
  • Comfortable with electronic documentation and structured care planning.

Also useful

  • Experience with dementia informed care.
  • Background in post discharge transitional care.
  • Familiarity with PIPA / PIPEDA privacy obligations.

Compensation
