About the role
We're hiring a Registered Nurse to lead in home clinical work for our Ageing in Place and Home Care programs. You'll work alongside a Monarch physician and a small care coordination team, conducting comprehensive assessments, building plans with families, and providing the steady follow up that keeps proactive care actually proactive.
Responsibilities
- Conduct comprehensive in home clinical and functional assessments.
- Lead medication reviews and reconciliations with the client's pharmacy.
- Document and update written care plans in the household record.
- Coordinate with the family physician, specialists, and the client's family, with consent.
- Provide ongoing nursing follow up at the cadence each plan calls for (typically monthly or quarterly).
- Mentor caregivers staffed on the household and review shift documentation.
Requirements
- Active registration in good standing with the BC College of Nurses and Midwives (BCCNM).
- Current professional liability coverage.
- Three or more years of relevant nursing experience (community, home care, geriatrics, or primary care).
- Valid BC driver's licence and reliable vehicle (visits across Greater Victoria).
- Comfortable with electronic documentation and structured care planning.
Nice to have
- Experience with dementia informed care.
- Background in post discharge transitional care.
- Familiarity with PIPA / PIPEDA privacy obligations.
